How to Register

To register for training, you must:

1. Download the Program Registration Form SPB-46 (see below).
2. Complete the form.
3. Have the form signed by your training officer or training coordinator.
4. FAX the form to (916) 657-2502.

If you do not have access to a FAX machine, please mail the completed Program Registration form to:

If you do not have MS Word or Acrobat Reader, you can download a free viewer of your choice. If you do not have either software, please obtain the viewing software first--then download the appropriate formatted SPB-46 form.
