How to Register

To register for training, you must:

1. Download the SPB Training Registration Form, SPB-46 (see below).
2. Complete the form.
3. Have the form signed by your training officer or training coordinator.

    4.   If you do not have access to a FAX machine, please mail the completed SPB Training Registration form to:
